Medication Administration

All medications (prescription and non) must be transported to the school in their original packaging or pharmacy bottle by a parent. Students are not allowed to carry medication to school , in school, or on the school bus (other than an epinephrine auto-injector (EpiPen) and asthma inhalers if age appropriate). Prescription medications must contain the pharmacy label. The first dose of a medication should be given at home, if possible.
Prior to administration of any non or prescription medication, a parent must complete the following parent/guardian authorization form that can be found here: parent/guardian authorization form
This does not apply to over-the-counter medications held in stock in the nurse's office (i.e. Tylenol, Motrin) for as needed during the school day. This release will be found on the parent Emergency Information form sent home to every student at the beginning of the school year.
In addition to the above, those students with life threatening food allergies must also submit their physician's allergy and anaphylaxis emergency plan/school order or the school's FARE form found here: food allergy and anaphylaxis care plan
